Case management

As a qualified nursing and care professional you can also work in case management and support people in need of care by providing them with comprehensive information about available nursing and care services. When home care services are no longer sufficient, you will support clients in their transition to a residential care facility.

Graduate healthcare and nursing professionals working in case management provide service-oriented information and counselling to help elderly people and people needing nursing and care to stay in their usual environment for as long as possible. In your capacity as a case manager, you will visit clients at home to assess their individual needs and then arrange the appropriate services. Your goal is to accompany and support your clients in every situation to enable them to live a self-determined life.
